New Patient Checklist
Before you visit our office for the first time, make sure to pack the following items for a quicker check-in and better assistance.

Insurance Information and Photo ID
Dr. Maurer sees in- and out-of-network patients. Please bring your insurance card to your visit—regardless of coverage—to get registered and seen quickly. In addition, please bring a valid form of photo identification.

Lab and Current Test Results
If you’ve had any blood tests, scans, or other tests, bring in printed copies or ask the lab to fax Dr. Maurer’s office directly. The clearer picture we get of your current health, the better we can serve you.

Recent Notes from Previous Physicians
Ask your previous doctor to fax your medical records and notes to our office at 410-489-5527. Or, print your documents and bring them to your appoinement. Arriving with your complete health record will allow Dr. Maurer to get familiar with you and your health immediately, so he can customize a medical plan to keep you feeling great.

Immunization Records
No matter your age, please bring your vaccination records to your first visit. If you’re under 25, also bring your pediatric vaccination record. Faxed or printed copies are all we need to ensure you’re up-to-date on preventative care.
